BETTER HEALTH CLINIC

Diabetes Care plans and Management
The doctors and nurses at Nelson Plaza Clinic provide education in diabetes prevention, diagnosis, and advice in the management of diabetes. Some forms of diabetes can be controlled through diet and exercise, while other forms will require medication such as Insulin. The most common types of diabetes are Type 1 and Type 2.
Chronic Disease Management
Nelson Plaza Clinic provides care coordination and self-management support to help people with diagnosed chronic disease to better manage their condition and access appropriate services in order to improve health outcomes, prevent complications and reduce the need for hospitalisation.

Our Chronic Disease Management Program connects you with your regular GP, a nurse and may include care with appropriate primary, community and acute care services by:

  • Proactively identifying people most in need of and likely to benefit from the program
  • Undertaking comprehensive assessment
  • Supporting shared care planning
  • Delivering care coordination and self-management support services
  • Regularly monitoring and reviewing participants
Asthma Care Plans and Management
Nelson Plaza Clinic doctors and nurses can provide information, education, and treatment for patients who suffer from asthma to help increase the patient’s quality of life. Asthma is caused by the narrowing of the airways in the lungs, and symptoms can include wheezing, coughing (particularly at night), chest tightness, difficulty in breathing and shortness of breath. Asthma is a treatable health condition. Although at present there is no cure, with good management, people with asthma may lead normal, active lives.

Please discuss with your doctor the best way to manage your asthma and ensure that you discuss any concerns.

Smoking Cessation Management
Our smoking cessation program aims to help you quit or reduce your smoking habit in a non-judgemental environment. Our health professionals will provide you with advice, information and support in your journey to better health.
45 - 49 yr old Health Checks
The Australian Government offers this health check to patients aged 45 – 49 yrs old who are at risk of developing a chronic disease such as diabetes or heart disease. The aim of a health check is to help detect, lessen effects and most importantly prevent health issues. These checks provide your GP with an opportunity to look at your medical history, family history and lifestyle to find out if you are at risk.

Risk factors include, but are not limited to:

  • lifestyle risk factors, such as smoking, physical inactivity, poor nutrition or alcohol use;
  • medical risk factors, such as high blood cholesterol, high blood pressure, diabetes or excess weight; and
  • a family history of a chronic disease ( eg. cancer, glaucoma, heart disease, depression or diabetes)

What can a health check involve?

  • Updating your medical history and undertaking examinations and investigations as required
  • initiating interventions and referrals
  • providing advice and information; including strategies to achieve lifestyle and behaviour changes
  • follow up of any problems identified

When booking your appointment, please ensure to let reception know that it is for a 45 – 49 yr old health check, so the appropriate time is allocated for you and your Doctor.

75 yr and over Health Checks
An annual health assessment will identify ways that you can improve your health and well-being. For instance, it will help you by advising you on how you can maintain your independence at home and in your community.

General GP Services

Child and Adolescent/Teenager Health
Your children are the most precious people in your lives, and going to the doctors can be a daunting experience for some children. Our warm and friendly staff are dedicated to providing a safe and friendly environment to meet our patients needs.

All the team at Nelson Plaza Clinic understand the difficulties in your teenage years when you require personal and confidential advice. We encourage our adolescent patients to make appointments and feel comfortable and aware of the care and privacy that we offer.

Womens Health
Women’s healthcare may include but is not limited to regular PAP screening, breast checks, referral for mammography, family planning options, fertility counselling and management of menopause. Lifestyle plays an important role in all women’s lives, with healthcare focused on prevention to maintain optimum health.
Implanon Insertion and Removal
For Implanon insertion, our doctors require you make an initial appointment to go through the information and consent forms for an Implanon.

Removal of an implanon is only a 10-minute appointment.
If you are removing and replacing an implanon – a longer appointment will be required.

Mens Health
Men’s healthcare may include but is not limited to prostate screening, family planning options, screening pathology tests and sexual health. Lifestyle plays an important role in all men’s lives and with healthcare focused on prevention to maintain optimum health.
Skin Clinic
Nelson Plaza Clinic doctors are experienced in the detection and treatment of skin cancers.

Dr Angelo Fortune and Dr Dhei Abbod provide 20 minute skin clinic consultations for full body skin checks. If you are concerned at any stage about any changes to your skin e.g., moles, sun spots, solar keratosis, please make an appointment as soon as possible.

Immunisation and Travel Vaccines
Immunisation protects against a range of preventable diseases and infections. Childhood immunisations, catch up immunisations, are provided.

We do not keep travel vaccines in stock, so you will need to obtain a script for the pharmacy to order any vaccines required.

Mental Health
A holistic approach to healthcare is important, including physical, social, spiritual and mental health. Mental healthcare may include but is not limited to medication management, regular counselling, referral to allied health providers as well as preparation and ongoing management of GP mental healthcare plans
Indigenous and Torres Strait Islander Health

We would like to acknowledge and pay respect to the traditional owners of the land on which the Nelson Plaza GP Superclinic is built – The Worimi. We recognise the strength, resilience, and capacity of Aboriginal people in this land, and we pay respect to the knowledge embedded forever within the Aboriginal Custodianship of Country.

The staff at Nelson Plaza Clinic are committed to providing culturally safe health care services and welcome all Aboriginal and Torres Strait Islander communities to our clinic. All of our General Practitioner services are bulk billed for identified Aboriginal and Torres Strait Islanders.

Weight loss

Many patients have difficult with weight loss. Some of our doctors run specific clinics that can assist you with your goals. Call reception on 02 4981 2555 to book an appointment.

Antenatal and Postnatal Care

We are fortunate at Nelson Plaza Clinic to have a midwife at the clinic who can participate in shared-care for pregnant patients with our experienced GPs. Several GPs have additional qualifications from the Royal Australian College of Obstetrics and Gynaecology.

Postnatal care is also important. Our midwife also has additional qualifications in lactation and breast feeding.

Minor Procedures

We can provide a range of minor procedures for patients if required.

Workcover Consultations
Nelson Plaza Clinic offer workers compensation appointments. When you make your appointment, please advise the reception staff that you are booking for a workcover appointment, as this may influence the appointment length.

Worker’s Compensation consultations are billed at WorkCover approved rates. These consultations will be directly billed to your insurance company if they have accepted liability and if you can provide ALL billing details (including claim number, name and address of insurance company) at the time of consultation. If we do not have these details you will have to pay the full amount and claim that from the insurance company.
